- Latest Income Tax Return and/or Form 2316
- Proof of Billings
- Bank Statements
FOR EMPLOYED ABROAD:
- Certificate of Employment with Compensation
- Latest Crew Contract (for seamen)
- Latest six (6) months Proof of Monthly Remittances or payslips
- Special Power of Attorney authenticated by the Philippine Consulate
- Proof of Billings
